Behavioral Health Clinic Community Liaison/LPN (Full Time)

Department: Archer Administration
Location: Lake Charles, LA

DESCRIPTION OF POSITION:

  • The Behavioral Health Community Liaison serves as a liaison to area nursing homes to coordinate psychiatric and behavioral health physician consults. The position will also handle anything pertaining to nursing home rounds such as, but not limited to prior authorizations, prescription refills, and labs. The position will perform duties within the scope of practice, ensuring consistency with LCMH policies and procedures, standards of care, approved protocols and provider orders. The position will also adhere to practice standards governed by the Louisiana State Board of Nursing. This position directly reports to the Practice Manager.

RESPONSIBILITIES AND DUTIES:

  • Performs assessments and reassessments of patients.
  • Accurately documents according to hospital policy.
  • Possesses competency in use of technology and computers.
  • Monitors behavior, mental status and medication compliance; makes appropriate referrals.
  • Liaisons with patient, patient families, on-site departments and community agencies to coordinate current treatment and/or assist with aftercare planning.
  • Assists in the implementation of Quality Improvement; Quality Management (QI/QM) programs and the monitoring of compliance with required policies, procedures and standards.
  • Adheres to internal controls and reporting structure.
  • Assesses and documents a patient's mental health needs.
  • Conducts social history assessments and monitors mental health status of patients.
  • May administer and score psychometric tests under supervision.
  • Works with the assigned Psychiatrist to coordinate consults at area nursing homes.

EDUCATION AND WORK EXPERIENCE

  • Graduate of School of Nursing.
  • Current Louisiana Practical Nurse license or compact state license as Licensed Practical Nurse.
  • BLS-CPR certified.
  • 2- 4 years’ experience working with behavioral health conditions and/or nursing home population preferred.
  • Employee must be able to furnish own transportation to and from nursing home visits.
  • Must maintain active driver’s license and automobile insurance
  • Requires emotional stability and resilience; must be adaptable to change; be decisive and demonstrate forethought.
  • Must be receptive to delegated responsibilities and work within scope of practice.
  • Possesses basic problem solving skills.
  • Ability to communicate with patient, family, peers, and medical personnel.

  • Work requires a variety of physical activities including sitting, standing and walking.
  • The work environment may be physically confining.
  • Visual acuity required for a variety of activities.
