Clinical Case Management Discharge Planner

DESCRIPTION OF POSITION:

  • The Clinical Case Management Discharge Planner provides discharge planning to patients and families.

RESPONSIBILITIES AND DUTIES:

  • Determining which hospital or community resource should be included in the patients discharge plan.
  • Accurately assesses patient’s financial resources, makes referrals to community groups to provide assistance as needed.
  • Coordinates transfers to extended care facilities, group homes, or other acute hospitals as appropriate.
  • Documents discharge plan in medical records, completing final documentation within two working days of discharge.
  • Functions as a member of a multidisciplinary case management team.
  • Participates fully as a member of Clinical Case Management Department.
  • Participates in the patient satisfaction survey.
  • Participates in continuing education programs to maintain skills and social work licensure.
  • Effectively solves problems.

EDUCATION AND WORK EXPERIENCE

  • High School Diploma or GED equivalent, BS or BA preferred.
  • One year experience in social service field. Applicants in a health care setting will be given preference.
  • Ability to communicate with patient, family, peers, and medical personnel and community referral agencies.

Physical Demands/Work Environment

  • Work requires a variety of physical activities, including moving about within and outside of all hospital properties for long periods of time.
  • Must be able to respond quickly and effectively to emergency and non-emergent situations.
  • May be required to assist in controlling disorderly conduct or combative patients.
  • Must be able to exchange accurate information with patient, family, peers and medical personnel.
