AI Transition Coordinator

Department: Archer Institute - Psychiatric Services
Location: Lake Charles, LA

The Transition Coodinator is a  Master Level Clinician or Registered Nurse that is accountable for intake assessment of history, summarizing presenting symptoms, providing recommendations of disposition & plan of care for psychiatric patients in the Emergency Department; Works closely with the Emergency Room Team and the patient regarding assessment, recommendations, & disposition;  Patient and family education;  Participates in multi-disciplinary treatment planning & needs of patients/ family. The Transition Coordinator is accountable for audits of medical records for quality of patient care; participation in performance improvement and quality management process within the department; education of case management staff on treatment and discharge planning.  The Transition Coordinator serves as a member of the Psychiatric Service Leadership Team.

Education: Master's Degree in Social Work/ Psychology/ Social Services Field or Registered Nurse

Experience: Prefer 2- 4 Years Experience in working with mentally ill &/or addictions

License/ Certification: RN, LPC, MA, MSW, PLPC, LMSW &/ or LCSW, Crisis Response Training, CPR
