Referral Development Coordinator - Full Time - Days

Department: Physical Rehab
Location: Lake Charles, LA

DESCRIPTION OF POSITION:

  • The Referral Development Coordinator duties are designated toward increasing quality referrals and admissions to Rehabilitation Services, including management of marketing and business development, Responsible for coordinating patient consults/preadmission requirements in collaboration with the IRF Case Manager. In addition, assists with referral development for Memorial Home Health as allowed. Assist the post-acute clinical liaisons, social workers, case managers and department administrators with coordination of patient outreach, education, referrals, admissions and post-discharge feedback to referring physicians and clinics.

SUPERVISION: Reports directly to the Director of Physical Medicine

RESPONSIBILITIES AND DUTIES:

  • Develops a business plan for direct and indirect marketing needs.
  • Participates in public relations within the five-parish region.
  • Educates appropriate healthcare professionals about admission criteria i.e., nurse practitioners of physician clinics.
  • Discusses all consultation and referrals with covering and attending physicians.
  • Collaborates with IRF Case Manager for patient referrals/preadmission requirements.
  • Communicates the medical, social and therapy status of persons referred to the Rehab program.
  • Meets the patients and families prior to admit to determine needs and issue of both the patient and family.
  • Evaluates contacts for quality, appropriateness, and conversion to admissions.
  • Utilizes hospital resources as appropriate
  • Monitors referral source follow-up activities
  • Actively participates in hospital wide marketing team meetings when requested.
  • Coordinates marketing activities with LCMH Marketing director and Director of Rehabilitation Services.
  • Reviews program contacts, evaluations, and admissions regularly and identifies changes in referral patterns conversion rates and adjusts strategy accordingly.
  • Develops and implements annual departmental goals and objectives for overall marketing plan and for adapting to health care changes.

EDUCATION AND WORK EXPERIENCE

  • RN/LPN licensure required. Must hold a current license according to licensing guidelines.
  • Two years of clinical experience in a rehabilitation setting is preferred. Possess excellent written and very communication skills.
  • Maintain CPR certification.
  • Ability to read, analyze and interpret statistical and financial reports. Ability to respond to common inquiries or complaints from referral sources, customers, regulatory agencies, or members of the health care business community. Ability to effectively present information to members of management or public groups.
  • Capable of clear and succinct verbal and written communications; Possess a high level of patience and tenacity; Capable of rapid problem identification and resolution.
  • Requires emotional stability and maturity; Must be adaptable to change, be decisive, and demonstrate foresight; Exhibits organizational and leadership ability.

Physical Demands/Work Environment

  • The physical demands vary according to physical setting which may include driving short and long distances, standing, walking, bending, stooping or sitting for long periods of time.
  • The work environment may be physically confining.
  • Visual acuity required for a variety of specific activities. Must have normal hearing or utilize adaptive hearing devices.
